I have also seen these, one quite large. I do not know how to pinpoint my location in game though so that I could submit it. I did submit a 'stuck' report though so perhaps that will help.

if you press report bug, the location get send automaticly with the report. exactly were you stay. so best if you stay very close to the bug and report it. and say what is of or what you done to get the bug.

Upside down icons in inventory. Seems to be the same spaces. I also switched them into the bank and back and they still appeared upside down.
