Hello.
Riding a naked horse in third or fourth gear should involve a risk of falling (similar to having a riding skill below 20), especially when performing maneuvers or using weapons and spells. The lack of stirrups should make effective fighting and pursuit impossible. This would diversify the current boring full heavy or naked meta and give some purpose to light and medium horse armor.

Dont think it needs to make you fall off when you got riding skills but it is known to improve things for lancing and steadier aim on the move for horse archers IRL.
