If you can actually create an arena where people cannot attack people when partaking in the arena... You should consider putting in a betting system and/or wager system.
Now, the betting system is self explanatory.. You bet on who will win/lose.. Whatever, and you watch the fight.
The wager system is what i think would be more interesting.. A system where people can wager their gold.
Imagine, a huge guild versus another huge, betting all of their gold. The results would be devastating, and everyone would want to watch at such stakes. Maybe you have group fights, maybe they select a 'champion'.. Regardless, i think people would be really engaged by such a system.
Again, look at games like WoW, where their Hardcore scene became so popular, and their duels whereby, one person dies forever, losing everything.
You could put in high stakes wagers, have a gold amount set etc...
But yes...
You really need to tap into severe stakes more i think; especially where guilds are involved.. You have something of a safety issue with your marco scale PVP.
